What are the responsibilities and job description for the Intensive In Home Qualified Professional position at Easterseals PORT Health?

At Easterseals PORT Health (ESPH), we’re seeking a compassionate and dedicated full-time Qualified Professional to join our Intensive in Home (IIH). As a valued member of our collaborative and supportive team, you will work together to transform the lives of individuals by providing a comprehensive range of services that treat the whole person and empower patients to find their way forward.
What You’ll Do :
As a Intensive in-Home Qualified Professional, you will be responsible for providing crisis and case management and individual / family therapy to prevent the need for out of home placement for children with a diagnosed mental health disorder. As a Qualified Professional, you will deliver services in various environments, such as homes, schools, detention centers and jails (state funds only), homeless shelters, street locations, etc. You will also be responsible for coordinating the initial assessment and developing the youth’s Person-Centered Plan as well as diffuse crises, evaluate nature of crises, and intervene to reduce the likelihood of a recurrence.

This full-time position offers a flexible 40-hour nontraditional schedule and will serve the Iredell County and Davie Counties. We offer competitive benefits to benefits eligible positions.
