POSITION SUMMARY:
The Community Health Worker (CHW) will be responsible for helping patients and their families to navigate and access community services, other resources, and adopt healthy behaviors. The CHW supports providers and the Chronic Care Management Coordinator(s) through an integrated approach to chronic care management, community outreach, coordination of services and follow-up. The CHW will actively promote, maintain, and improve the health of patients and their family through social support and informal counseling, advocate for individuals and community health needs, and provide services (i.e. blood pressure and blood glucose screening, etc.). Promote wellness by providing culturally appropriate health information to patients/families and care teams, education patients on disease prevention, conduct health related screenings and facilitate with other support groups.

Please note that actual pay will be determined based on relevant experience, skills, knowledge, abilities, education, licensure and certifications, and other business and organizational needs. Please also note this range is applicable for employees who reside in California only. A geographical pay differential may be applied for remote employees who reside out of state. - California Driver’s License
- CPR (American Heart Association)
- Knowledge of Microsoft software (Word, Excel)
- Language proficiency – English and Spanish
