Hospice Experience Preferred

LVN is responsible for providing one-on-one skilled nursing care and treatment to patients in their home. The LVN will provide comprehensive and patient specific care to promote their health and independence. The ideal LVN should embody Optimism, Enthusiasm and a Growth Mindset.

As an LVN, you will:

• Provide scheduled nursing visits to patients on Mondays through Fridays, day shift.
• Coverage Areas Available: Inland Empire region.
• Under the direction of the RN, supplement the nursing care needs of the patient as provided by the RN and as specified in the Plan of Care.
• Assist the patient in learning appropriate self care techniques.
• Complete appropriate clinical and progress notes, documenting outcomes of interventions.
• Provide services in accordance with organizational policies and procedures and regulations that define scope of practice.
• Assist the RN and/or physician in performing specialized duties related to end-of-life care.
• Prepare equipment and materials for treatment adhering to aseptic technique as required.
• Assess patient/family response to care, ensuring communication of information to appropriate team members.

Requirements

• Current State LVN License
• Valid Driver's License
• Valid Auto Insurance
• AHA CPR Certification
• Home Care/Hospice Care (1 year preferred)
• Healthcare experience (1 year required)

Job Type: Part-time

Pay: $3,000.00 - $4,000.00 per month
