Job Summary
The EHSS Administrator is vital in supporting the office and service programs delivered. This position will manage the office's daily activities, office access and security, source referral, complete intake activities, provide ongoing monitoring of people, and ensure the highest quality of service while meeting compliance expectations.
Responsibilities
- Provide administrative support to DD, Probation, CFS, and Foster programs as needed
- Communicate with the State Service Coordinators to ensure eligibility/authorization for funding of services/supports is available.
- Gathers, interprets and operationalizes information about the individual and makes it available to the agency’s team.
- Obtains all necessary consents, and completes a social history and other agency-required forms/paperwork, records, and documentation.
- Facilitates the completion of required assessments, services and safety plans, residential agreements, and coordinates with the individual’s team to deliver service, medical, and behavioral support.
- Maintains individual and agency records and ensures all electronic and physical records contain current information.
- Identifies and coordinates the individuals’ clinical and service needs and ensures that there is a coordinated plan of care with the agency and external agencies/professionals.
- Communicate with the agency Nurse or Managers regarding health concerns (e.g., missed appointments, medications, diagnoses, etc.) and take appropriate action.
- Identifies individuals with complex medical issues and provides appropriate agency and external interventions by qualified health professionals.
- Serves as a liaison to external agencies/professionals in collaboration with the coordinator.
- As part of the agency’s leadership team, provides input into the development of individual service plans and implements policy and procedures.
- Manage referrals within Therap system daily
- As Lead DSP, this role provides leadership to the EHSS DSPs
- Coordinate staff scheduling
- Manage appointments as needed
- Manage case communication with state correspondents
- Assist with home assessments as needed
Qualifications
- Bachelor's degree in human services, social work, psychology, or a related field. Equivalent experience may be considered.
- Strong interpersonal and communication skills.
- Ability to build rapport with individuals from diverse backgrounds.
- Excellent organizational and time management skills.
- Ability to work independently and as part of a team.
- Valid driver's license and reliable transportation.
- Must pass required background checks.
- Bilingual skills (English/Spanish) preferred.
- Trained in trauma-informed care, recovery principles, and crisis management
