VALOR - CASE MANAGER
The Case Manager is responsible for an assigned caseload of dually diagnosed, justice involved individuals needing mental health competency restoration supports. The Valor Program is a statewide program designed for clients who need or have a history of needing forensic and competency restoration services. The primary responsibilities will be to provide direct care services to restore competency by assessing treatment needs, developing, monitoring, evaluating treatment plans and progress, and facilitating interdisciplinary approaches. This role requires good judgement and flexibility in responding to crisis to follow up with supportive services to participants, provide focus and directed oversight, communication, and coordination. This is a 24/7 program and may require night and weekend coverage for the safety and well-being of all clients residing in the facility. Shifts may change to meet the requirements of daily operations. May be required to travel to attend limited meetings with contracting agencies.
ESSENTIAL DUTIES AND RESPONSIBILITIES

- Provides assessment of daily living skills (ADL’s), coordination, monitoring, planning, referral, and follow-up with comprehensive community services and supports.
- Assists with resource acquisition and assistance with application for benefits and entitlements in collaboration with Benefits/Navigation Team.
- Coordinates and monitors the delivery of services to clients, including medical, social, educational, and vocational services.
- Assists clients with the securing basic needs such as housing, food, clothing, medical and dental care, legal, vocational, and psychiatric services.
- Connects clients to community based and internally based resources, such as transportation services, peer supports, work programs, and navigation support.
- Practices trauma informed care.
- Collaborates with other service providers and community resources to ensure clients receive comprehensive services.
- Participates in case conferences and team meetings to ensure effective integrated service delivery.
- Maintains accurate, timely and up-to-date client records and documentation.
- Practices Evidence Based Practices (i.e.: IDDT, DBT, CBT, psychoeducation, Motivational Interviewing etc.)
- Provides crisis intervention and support to clients as needed.
- Perform other duties as assigned.
EDUCATION/ WORK EXPERIENCE:
Bachelor’s degree from an accredited university with a major in psychology, sociology, social work, counseling, or other related human services field required.
