What You’ll Do:
- Provide supportive services to adults with mental illness living independently in the community. Services include community linkages, assistance with health and wellness related issues, and socialization opportunities.
- Coordinate care with clinical teams and external agencies.
- Help consumers sustain wellness and recovery beyond program enrollment.
- Contribute to service improvements and consumer education.
- Ensure compliance with licensing and regulatory standards.
Job Requirements:
- Bachelor’s Degree in Social Work, Psychology, or related field from an accredited college or university.
- At least 2 years of experience with community-based outreach services assisting mental health consumers preferred.
- Strong computer skills (Word, Excel).
- Valid Driver’s License and acceptable driving record.
- Excellent communication, planning, and organizational skills.
