Job Position : Case ManagerLocation : Pontiac, MI 48342Pay Rate : $20.75Position Summary : We are seeking a compassionate and dedicated Case Manager to join our team. In this role, you will provide essential support services to individuals and families experiencing homelessness. You will play a crucial role in helping clients achieve housing stability and long-term self-sufficiency.Responsibilities : Manage a caseload of 20-30 clients, conducting needs assessments, developing action plans, and monitoring progress.Assist clients with housing searches, applications, and move-in processes.Provide ongoing case management support, including benefits navigation, resource referrals, and life skills development.Conduct HMIS data entry and maintain accurate client records.Collaborate with internal and external partners to coordinate services and ensure client success.Participate in team meetings, trainings, and community events.Adhere to all agency policies and procedures.Qualifications : Associates degree preferred; Bachelor's degree in Social Work or related field preferred.1-2 years of experience in case management or a related human services field.Strong understanding of homelessness and poverty issues.Experience working with diverse populations.Excellent communication, interpersonal, and organizational skills.Proficiency in Microsoft Office Suite and HMIS.Valid driver's license and reliable transportation.#troy
