Mental Health Service Coordinator
Job Description:
This position is unique in regard to service coordination as it blends the responsibilities of a service coordinator and a mental health professional into one role. This person would not only be responsible for facilitating and coordinating long-term services and supports (LTSS), but also ensuring that their clients’ mental health (MH) needs are being addressed. Examining both the physical and mental health needs of a client will require an individual capable of performing holistic person-centered assessments to capture all areas of need.
Duties and Responsibilities:
· Responsible for working with individuals diagnosed with a serious mental illness who are also eligible to receive LTSS.
· Perform holistic clinical assessments to determine the needs of a participant as well as make a preliminary MH diagnosis as necessary while examining Medically Necessary Criteria for the most appropriate level of care to best service the client.
· Collect additional necessary information including the client’s preferences, strengths, and goal to work towards the development of a Person-Centered Service Plan.
· Refer clients to most appropriate level of service based on information collected during assessments. Referrals to services may include both LTSS and MH supports.
· Actively coordinates with the client and service providers to ensure seamless coordination between physical, behavioral and support services.
· Assist clients in obtaining Home and Community Based Services that will promote independent living.
· Explore coverage of services to address the participant’s identified needs through other sources, including services provided under Medical Assistance, Medicare or private insurance and other community resources.
· Identify, coordinate, and assist participants in gaining access to needed LTSS and Medical Assistance services, as well as non-Medicaid funded medical, social, housing, educational, and other services and supports.
· Continuously monitor LTSS and MH needs for participants.
Qualifications:
· Masters-level Mental Health Professional with major course work in social work or counselling, OR
· Masters-level Mental Health Professional with major course work in sociology, social welfare, psychology, and other related social sciences
· In addition to these requirements, SCs employed by SCE and assigned to UPMC for the Pilot Program will meet the following preferred qualifications to the extent possible:
- Licensed Social Worker, Licensed Clinical Social Worker, Licensed Professional Counselor.
- Demonstrated experience working with individuals with chronic and persistent mental illness.
