Erie Family Health Centers, a top workplace in Chicago and suburbs, is looking for a valuable addition to our Case Management team! The Case Manager will work with medical providers and nursing staff to identify potential clients, assess, and deliver supportive services to clients.

Compensation is based on each candidate’s experience, skills and education within the range identified for the role. Candidates who meet the minimum requirements of the role will start at entry in the range. Any additional skills, experience and education will be reflected in the compensation offered.
Main Duties & Responsibilities
- Conducts outreach, advocacy, assessments, care plan, home visitation, and EMR data entry.
- Collaborates with fellow Case Managers to provide constant floor coverage throughout the day.
- Displays familiarity and knowledge of the care plan of each client and facilitates referrals and follow-up process.
- Follows the Department of Human Services Maternal and Child code requirements.
- Acts as a mandated reporter to the Department of Children and Family Services
Qualifications
Education
- Bachelor's Degree required in social work, child development, community health, or related field is required.
- Current CPR is required.
Skills and Knowledge
- Minimum of two years' work experience in health, social or community services preferred.
- Bilingual (English/Spanish) required (May be required to assist or work with non-English/Spanish only speaking patients and/or guests)
- Local travel required, valid Illinois Driver’s License and automobile insurance also required.
