Role Description
This is a full-time on-site role for an RN Clinical Care Coordinator at Evergreen Private Care in Stuart, FL. The RN Clinical Care Coordinator will be responsible for conducting patient assessments, developing care plans, coordinating care services, educating patients and families, and ensuring compliance with regulatory standards. Additionally, the coordinator will collaborate with healthcare professionals and support staff to optimize patient care.
Qualifications
