Duties
- Conduct assessments to determine client needs and eligibility for various social services programs, including Medicare and Section 8 housing assistance.
- Provide crisis management support to clients facing urgent situations, ensuring their immediate safety and well-being.
- Assist clients in accessing behavioral health resources and services, offering guidance throughout the process.
- Mentor individuals in developing life skills and coping strategies, fostering independence and resilience.
- Supervise and support clients in childcare settings, ensuring a safe and nurturing environment.
- Collaborate with social workers and other professionals to develop comprehensive care plans tailored to individual client needs.
- Maintain accurate records of client interactions, progress notes, and service referrals in compliance with confidentiality regulations.
- Stay informed about community resources and programs available to assist clients effectively.
Experience
Candidates should have experience or knowledge in the following areas:
