Success Coach for El Puente Program
Families and Youth Innovation Plus is hiring a full-time Success Coach El Puente Program.
The Success Coach provides specialized case management services to individual client needs. They also create programs to benefit clients and educate clients on available community resources. Success coaching ensures that clients receive LVP services in a timely, appropriate, efficient and coordinated fashion.

Key Responsibilities
- Provide clients with supportive services
- Coordinate services between providers and clients
- Interpret agency services to clients and community
- Provide case management services and skill-building services to youth in the program
- Provide housing case management services
- Maintain individual case records on clients receiving services
- Provide specialized case management services to individual client needs
- Educate clients on available community resources and services
- Create programs and services to benefit clients and the community
- Attend community meetings relevant to program and clients
- Administer and facilitate the completion of appropriate intake forms, assessments, intake packet, and
- service plans
- Conduct field visits, attend meetings and be available to deliver services during traditional and nontraditional
- hours (when needed) in; the client's home, educational institutions, places of employment, FYI's Main office, JPPO offices, detention facilities, reintegration centers, or any other setting that enables and promotes, the consistent and structured delivery of LVP services
- Organize and facilitate Life Skills Groups, as needed, in both individual and group settings
- Required to participate and interact in youth activities (for example: camping, sport activities, overnight youth conferences, etc.), which may involve occasional overnight activities and trips
Qualifications and Experience
- Bachelor's degree in Social Work, Family and Consumer Sciences, Criminal Justice, Psychology,Sociology, or Criminology from an accredited College or University.
- Must pass a CYFD Criminal Records Check
- Must maintain a full certification of the following trainings; CPR/First Aid and CPI
- Must maintain a valid NM Driver's Licenses.
