DEPARTMENT: Empowerment Center

REPORTS TO: Program Manager

Salary Range: $19.00 - $20.00

The Family Assistance Program empowers all individuals and families, regardless of age or gender, by providing the knowledge and skills to live healthy, safe, and fulfilled lives.

GENERAL JOB DESCRIPTION

The Case Manager is an individual contributor to a team that services adult clients. The daily tasks include informing clients about care plans, promoting advocacy, and resource referrals. Upon entry into the program, an intake assessment of all clients will be conducted. Case Managers work with clients to set goals, improve self-development, and apply for benefits.

MAJOR DUTIES AND RESPONSIBILITIES

· Meeting with clients weekly, creating and maintaining individual client goals.

· Assisting clients in accessing community resources and services.

· Facilitating communication between clients and service providers.

· Manage care plans: Assess clients’ needs, develop tailored care plans, and make necessary adjustments to improve outcomes.

· Liaising with multidisciplinary teams: regular communication with the shelter manager and other professionals is essential to coordinate care plans.

· Ensure that all documents submitted on behalf of a client are valid.

· Documenting progress: Recording clients’ progress, including referrals, shelter visits, and other interactions, is vital for tracking and improving care.

· Identify the level of need for health and welfare, social skills, and educational support.

· Coordinating with external services: work with community services, insurance companies, and other external entities to ensure clients receive comprehensive support.

· Responding to emails and voicemails: address emails, voicemails, and security concerns appropriately and document observations and actions taken.

· Travel between offices, units, and business locations during the workday.

PHYSICAL REQUIREMENTS

The employee must be able to sit and stand for long periods, Handle, grasp, and feel objects and equipment, lift up to 50 pounds, reach with hands and arms, extend both arms above the head and/or go below the waist, bend or squat, walk and climb stairs and ladders, hear in a moderately noisy environment, and use a phone or headset for extended periods.

Work Conditions: Most of the duties for this position are performed indoors, although travel between work sites may be required. The incumbent may be exposed to some noise (electrical, mechanical, or human) on an occasional basis, as well as other related hazards associated with an office environment.

MINOR DUTIES AND RESPONSIBILITIES

· Input client data into CAP360

· Provide all required documentation, including client follow-up, outcome, and evaluation.

· Identify repairs and/or household needs and coordinate with maintenance or others to ensure needs are met.

· Complete and maintain case notes and document service statistics.

· Facilitate resident's safety by doing periodic inspections, teaching fire safety, and conducting regular fire drills.

· Attend community resource meetings to learn potential resources.

· Advocating for clients’ needs.

· Other duties as assigned.

QUALIFICATIONS FOR THE JOB

Education:

A bachelor’s Degree in mental health, social services, or business is preferred.

A high school diploma, equivalent (G.E.D.), or associate of arts degree is accepted.

Experience:

Knowledge of social work practices with high-risk populations.

Experience working with people with severe mental illness

Excellent Documentation skills.

Excellent written and verbal communication skills.

Experience with crisis intervention and coordinating external service.

Ability to motivate and establish rapport with at-risk youth, transitional youth, and adults.

Other:

Bilingual Preferred

Basic Computer Skills (Data Entry, MS Office)

Valid Driver’s License

Proof of Auto Insurance and Valid Registration

Have a good driving record (no more than 2 points)

Job Type: Full-time

Pay: $19.00 - $20.00 per hour

Expected hours: 40 per week
