Targeted Case Management Position
FamilyCare Counseling Center is looking for a Case Manager with a Bachelor Degree in psychology, social work, or mental health; Minimum one (1) year of experience post degree working with persons in a human services setting. Must provide own transportation to provide home visits.
SKILL SETS: Case Management for FCC must have the ability to provide adequate communication skills, process verbal and written instructions, be dependable, interact effectively with the public and staff, adaptability with being able to deal with frequent change, delays or unexpected events, and a strong sense of commitment. Case Management requires excellent communication, interviewing, documentation and organizational skills as well as the proven ability to efficiently use a computer and communication systems.
PHYSICAL AND ENVIRONMENTAL DEMANDS: Visual acuity necessary for updating files and completing documents. Requires frequent kneeling and crouching to file records. Hearing and talking is necessary to answer phone and talk with employees and other individuals. Work is light with walking, standing and sitting. Manual dexterity is required to perform work processing and other clerical duties. Possible exposure to bloodborne pathogens, biohazardous waste, hazardous waste and communicable diseases.
JOB SPECIFIC FUNCTIONS:
● Must attend approved TCM trainings specific to the assigned targeted population.
● Attend trainings annually that provide a certificate of a minimum of 6 CEUs specific to the targeted population being served.
● Conducts home visits to assess service coordination needs for clients.
● Conducts and prepares written needs assessments for assigned clients
● Assists in development of client service plans
● Coordinates and arranges for needed services as identified in treatment plan
● Assists clients in accessing all needed services within the community
● Performs follow-up on services to assure they meet client needs
● Educates clients and their families on the value of treatment
● Performs advocacy on behalf of clients
● Provides case consultations to service providers
● Frequent Supervision meetings with Supervisor
● Performs other duties as assigned.
Education/Experience:
● Bachelor's (Required)
