Responsibilities:
- Deliver high-quality patient care through direct contact and mediated communication.
- Conduct thorough physical examinations and assessments for patients of all ages.
- Perform chronic care management for patients with conditions such as hypertension, diabetes, and asthma.
- Administer annual wellness visits for adults and pediatrics, including vaccinations and preventive screenings.
- Collaborate with physicians and other healthcare professionals to develop comprehensive patient care plans.
- Promote patient engagement, health literacy, and shared decision-making.
Requirements:
- Nurse Practitioner (NP) / Physician Assistant (PA) with 2 years of experience in a direct primary care setting.
- Strong understanding of Patient-Centered Medical Home (PCMH) principles and value-based care.
- Ability to perform pediatric exams and see children ages 3 years and up.
