General Description of Position : The primary responsibilities of the QP / Certified Peer Support Specialist are to promote whole person, integrated care by planning, coordinating, tracking, closing of gaps in care and monitoring care to individuals to achieve the key goals of the behavioral health plans. Certified Peer Support Specialists also provide essential expertise and consultation to the entire team to promote a culture in which each individual’s point of view and preferences are recognized, understood, respected and integrated into treatment, rehabilitation, and community self-help activities. This position may serve as an advocate for the person supported. Performs a wide range of tasks to assist the people supported in regaining control over their own recovery processes. These interventions are strength-based and focused on promoting recovery, symptom reduction, increased coping skills, and achievement of the highest level of functioning in the community. Functions as a role model of competency in recovery and ongoing coping skills.
Duties and Responsibilities :
You will report directly to the Clinical Director. Your responsibilities will be :
- Abide by federal, State, and local standards, as well as abide by the highest ethical
- standards as listed in the Peer Support Code of Ethical Conduct.
- Modeling effective coping and self-help techniques to individuals
- Providing empowerment skills and support in successful recovery living
- Assisting individuals in finding resources, advising of processes, and encouraging follow through with proposed resolutions, locating social activities, or provide other assistance as needed
- Supporting / assisting individuals in daily decision making and resolution of minor problems
- Facilitating social opportunities in the community
- Sharing personal story of recovery and resilience to instill hope
- Create and manage PCP's and Authorizations. (Excluding submissions of authorization).
- Billing for PCP's.
- Responsible in making sure all intakes, and assessments are up to date and in compliance.
- Responsible in checking insurance at the beginning of the month for Peer Support clients.
- Supervise that documentation and policy notes are completed and submitted within 24 hours after contact with consumer.
- Attend monthly peer support supervision meeting.
- Follow all State, local, and federal guidelines, related to duty to warn, duty to report, and protection of confidentiality and client rights.
- Participate and complete all required agency trainings and meetings, as well as all required care management-based trainings from the State, Technical Assistance entity or payor and within required timeframes as assigned.
- Other duties as assigned.
