About the Role :
The Population Health Manager will work under the supervision of the Vice President of Revenue Operations to provide support and assistance in the implementation and maintenance of population health management programs and strategy. This position is responsible for helping to monitor and meet various population health goals across the organization. This role will have direct responsibility for identifying, improving, and maintaining population health metrics related to various payors and programs.
Key Responsibilities :
- Acts as the point of contact for status reports, priority setting, and coordination of resources to meet outcomes for payors and programs.
- Assist in the development and implementation of population health management programs.
- Participate in the identification and stratification of patient populations based on risk factors and health needs.
- Provide patient outreach and engagement support to promote health behavior change and self-management.
- Conduct patient assessments and assist with the development of care plans.
- Collaborate with clinical staff to ensure patients receive appropriate care and services.
- Provide data analysis and support the use of data to drive quality improvement initiatives.
- Attend and participate in interdisciplinary team meetings and case conferences.
- Ensure patient confidentiality and privacy is maintained at all times.
- Other duties as assigned by the Vice President of Operations.
