Job Summary:
Oaks Medical Group is seeking a Population Health Coordinator to support our primary care providers in managing value-based care contracts with insurance companies. This role focuses on improving patient outcomes by coordinating care, managing population health initiatives, and ensuring patients receive appropriate preventative and chronic disease care. The ideal candidate will be proactive, detail-oriented, and passionate about enhancing patient health outcomes.
Key Responsibilities:
- Collaborate with primary care providers to help close care gaps for patients.
- Conduct outreach to engage patients in preventative care, chronic disease management, and wellness programs.
- Monitor patient progress, identify barriers to care, and coordinate follow-ups to improve health outcomes.
- Utilize electronic health records (EHR) and population health management tools to track patient data and measure progress.
- Support quality improvement initiatives and ensure compliance with value-based care requirements.
- Assist in tracking key performance indicators (KPIs) related to value-based contracts and population health goals.
- Work closely with care teams to enhance patient engagement and optimize care delivery.
Qualifications & Skills:
- Medical Assistant (MA) or Certified Nursing Assistant (CNA) preferred.
- Knowledge of electronic health records (EHR) and population health management tools preferred.
- Understanding of value-based healthcare, Accountable Care Organizations (ACOs), and population health strategies is beneficial.
- Strong communication and interpersonal skills with a patient-centered approach.
- Excellent organizational skills and ability to manage multiple tasks independently.
- Proficiency in computer applications and willingness to learn new healthcare software.
Job Type: Full-time
Pay: $18.00 - $20.00 per hour
Expected hours: 40 per week
