Per Diem Homecare Nurse

We are currently looking to add a per diem RN Field Nurse to our team at our private pay home care agency. This person will join a dynamic group, who strive to provide exceptional service to our clients throughout Westchester County and occasionally Putnam and the Bronx, in the comforts of their own homes. Responsibilities and Duties

• The initial client assessment to include patient assessment and the home environmental assessment
• Develops the plan of care and performs supervisory visits in the client’s place of residence.
• Daily case management, including compliance with all NY LHCSA regulatory standards
• Supervision of paraprofessional staff
• Delivery of quality aide services along with support in the recruitment and retention of staff

Qualifications and Skills

• RN currently registered and licensed to practice nursing in NY
• Experience with NY state LHCSA regulations
• Experience in dealing with Dementia is a plus
• Previous home and hospice care
• Experience in Home Health Aide training
• The applicant should be able to demonstrate maturity, professionalism and the ability to deal effectively with the demands of the job
• Must have excellent telephone and interpersonal skills.
• Must be able to navigate difficult / challenging situations with professionalism
• Must have some office administrative experience, specifically filing and organizational, with both paper and electronic files.
• Excellent computer and MS office skills.
• The applicant will be required to submit a complete background check, including a check of criminal and driving records along with fingerprinting and substance testing where required
• The applicant must have Valid Drivers License and a safe and reliable car
• The applicant will be required to provide an APE and PPD/Quantiferon results within the past 3 months and MMR titres.

Job Types: Part-time, Per diem

Pay: $35.00 - $55.00 per hour
