Job Summary:
The Peer Provider – Outreach and Engagement serves as a frontline advocate, supporting individuals experiencing homelessness, substance use disorders, and/or serious mental illness across the Finger Lakes region. This mobile position focuses on relationship-building, street outreach, and recovery support, with an emphasis on guiding individuals through systems of care and helping them navigate the path to housing, recovery, and stability.
Working under the direction of the Sr. Director of Crisis Services and Mobile Clinic—with collaborative support from the Housing and Employment Manager and Connections Regional Coordinator—the Peer Provider offers meaningful engagement, harm reduction support, and peer-led recovery services both in the field and across FLACRA service areas.
Skills & Experience:
- Strong interpersonal and written communication skills
- Ability to work effectively with diverse populations and community-based providers
- Understanding of recovery-oriented care, peer support philosophy, and trauma-informed engagement
- Skilled in time management, multi-tasking, and problem-solving in fast-paced environments
- Comfort working in community-based and field settings, including outdoor and non-traditional environments
- Competency in crisis intervention techniques and early engagement strategies
- Ability to remain calm under pressure and de-escalate crisis situations
Key Responsibilities:
- Conduct street outreach to individuals experiencing unsheltered homelessness, providing immediate peer support, harm reduction resources, and warm referrals to crisis services, shelters, and treatment programs
- Build trust with clients by offering consistent, non-judgmental engagement and peer-based support throughout their recovery journey
- Coordinate with Continuum of Care (CoC), DSS, and Coordinated Entry teams to support individuals experiencing housing instability
- Distribute food, hygiene kits, and weather-appropriate supplies during outreach to meet basic needs and foster connection
- Serve as a liaison between individuals and service systems, helping clients navigate the housing, behavioral health, and recovery support networks
- Participate in interdisciplinary treatment team meetings and contribute to person-centered recovery plans
- Facilitate psychoeducation on topics such as illness management, coping strategies, harm reduction, and family engagement
- Support clients in developing psychiatric advance directives and personal crisis plans
- Provide case management assistance, service coordination, and resource brokering to remove barriers to recovery
- Act as a recovery mentor by modeling resilience, healthy coping skills, and long-term wellness strategies
- Maintain accurate and timely documentation in compliance with FLACRA standards
- Advocate on behalf of clients and families, identifying and addressing service gaps or access issues
- Assist clients with accessing detox, treatment, support groups, housing applications, and other recovery-related services
- Participate in training and certification maintenance requirements, including continuing education
- Perform other duties as assigned in support of program goals
