Fort HealthCare / Fort Medical Group-Lake Mills Clinic is seeking a BC / BE Family Medicine physician for an outpatient practice in Lake Mills, WI :

  • Full-time; 32-patient contact hours per week
  • Join Advanced Practice Provider team
  • Outpatient call, telephone only in a robust call group of 1 : 12, roughly three days of call per month

  • Multi-specialty group practice with satellite clinics offering primary and specialty care in Fort Atkinson, Cambridge, Jefferson, Johnson Creek, Lake Mills, and Whitewater.
  • Consists of 33-physicians and 17-advanced practice practitioners in multiple specialties conducting clinics in Jefferson County and surrounding communities.

  • Fort Memorial Hospital, a 49-bed not-for-profit, independent community-based hospital serving a regional population of close to 90,000.
  • ED staffed 24 / 7 by EM-residency trained physicians and advanced practitioners.
  • Adult hospitalist program.
