Title : Senior Case Manager
Unit : Women Rising / Family Services
Reports to : Program Supervisor
Status : Full Time; Regular; Non-Exempt
Salary Range : $27.48 to $30.22 (approximately $50,000 to $55,000 annually)
Location : Long Island City
Days / Hours : Varies : Monday through Friday, 9 : 00am - 5 : 00pm, 11 : 30pm-7 : 30pm (Subject to change depending on program needs)
Position Summary :
The Senior Case Manager will be responsible for case management duties, facilitating women's parenting workshops and overseeing the day-to-day program operations for the Women Rising Program.
Essential Duties and Responsibilities :
- Assist with program intake and assessments.
- Perform general case management duties, including individual sessions with clients enrolled in parenting.
- Assist clients in identifying and prioritizing needs;provide internal and external referrals.
- Facilitate women's groups using evidence-based curriculum.
- Oversee program planning and implementation.
- Ensure accurate and timely data entry of all information into the agency's database.
- Maintain and compile accurate data; and submit monthly / quarterly reports.
- Develop and maintain optimum collaboration among departments / programs and external agencies.
- Participate and conduct meetings and conferences.
- Engage in ongoing professional development.
- Perform other related duties as required.
