Responsibilities
- Complete admissions and evaluations, provide social services or referrals, and maintain resident and family communication
- Assist guests with financial and legal matters, such as pension applications and lawyer referrals
- Provide discharge planning services, including waiting list placement and home care arrangements
- Arrange counseling services and psychiatric medication management as needed
- Maintain accurate and timely documentation in MDS care plans
Requirements
- Current social work licensure in the state
- Bachelor's degree in social work required; master's degree preferred
- One year of supervised social work experience in a healthcare setting preferred
