Children’s Targeted Case Manager – Position involves working with and coordinating care for children and adolescents impacted by severe mental illness and their families. Selected candidate should possess strong communication, organizational skills and heightened attention to detail. Must be able to assess and function in a strengths based model of care, and be able to link patients with resources, coordinate appropriate treatment and provide documentation for the clinical record.
Bachelor’s Degree or equivalent combination of education and work experience in the Human Services Field required.
Hourly position: starting wage is $44,000/year -$21.15/hour.
