Social Services Director Position Summary
The Social Services Director plans, organizes, supervises, and directs all administrative and operational activities of the Social Services Department. Their leadership role includes the following responsibilities:
- Oversee the development, implementation, supervision, and ongoing evaluation of the Social Services Department.
- Meet with and assist residents and their families with medically related services, grievances, or other placement needs to help maintain the highest quality of care.
- Complete assessments and participate in comprehensive care plans.
- Facilitate residents’ safe transition upon discharge as well as facilitate transportation for appointments or other scheduled events.
- Maintain confidentiality of protected health information, including verbal, written and electronic communications.
- Perform all other assigned tasks as directed by the Administrator, Director of Nursing, or corporate team.
Social Services Director Requirements
- A minimum of a bachelor’s degree in social work or another human services field such as sociology, gerontology, special education, rehabilitation counseling, or rehabilitation.
- 1 year of supervised social work experience in healthcare
