What are the responsibilities and job description for the Community Hlth Outreach Worker position at Frederick County Health Department?
This position is in the learning capacity to deliver the Healthy Hearts program, to conduct community outreach/recruitment, conducts social determinants of health (SDOH) assessments with referrals and follow-ups and provides heart health education to clients of a local food program and the Breast and Cervical Cancer program (BCCP). Services are provided in both English and Spanish, as able.
