• Facilitate access to community resources, home health services, and vocational rehabilitation programs to promote patients' independence and quality of life.
Qualifications
• Master's degree in Social Work from an accredited institution.
• State temporary license, Registered Social Worker Clinical Intern, Associated Social Work license or Provisional LCSW License (if required by state).
• Ability to work effectively in a fast-paced environment and prioritize multiple tasks simultaneously.
• Strong communication and interpersonal skills, with the ability to build rapport with patients, families, and healthcare professionals.
