This is an exciting opportunity to join our team as a Social Worker with Fresenius Medical Care. As a key member of our nephrology healthcare team, you will play a vital role in providing psychosocial services to patients with end-stage renal disease (ESRD).
Responsibilities:
- Assess patients' psychosocial status and identify effective interventions to promote positive adjustment, rehabilitation, and improved quality of life
- Collaborate with patients, families, and healthcare teams to develop personalized care plans and address barriers to treatment
- Provide education and support to patients and their families regarding ESRD, dialysis, and related psychosocial issues
- Maintain accurate records and report quality indicators related to adherence and treatment outcomes
- Participate in interdisciplinary team meetings to discuss patient care and advance directives
The ideal candidate will possess:
- Masters in Social Work (MSW)
- State required license
- Strong communication and interpersonal skills
