Title: Human Service Worker 3 (HSW3) – Crisis, Respite and Residential Services
Direct Supervisor: Crisis and Respite Services Coordinator
Full/Part Time: Part time – Essential Employee
Computer Security: Level 5 – Access to paper & electronic records of adults actively receiving services
Scope of Job: Supports residents of 24-hour Compass Center services by providing them with resource and benefit information and providing direct support with obtaining resources and benefits during the resident’s stay in the program.
Job Duties:
1. Updates at the beginning of work shift by: a) emptying and reading all correspondence from personal mailbox; b) reading and responding to email and voice mail; c) reading all required updates including the team bulletin board; d) getting a working knowledge of the current situation on the team and interventions needed; e) obtaining and covering a cell phone, if assigned, and responding to calls/texts.
2. Establishes a professional, caring relationship with all people being served by the team. Has awareness of and is sensitive to each person's (and of co-workers’) cultural, racial, and ethnic background. Facilitates voluntary participation in activities and events related to the person's background. Focuses on each resident’s rehab planning and intervention services and each person’s health and safety. Responds to emergencies; assesses situations and plans immediate intervention; identifies people who are not doing well and provides interventions to address the situation; updates appropriate staff including Crisis and Respite Services Coordinator or manager on duty; reports critical situations.
3. Completes Rehabilitation Assessments and Individual Treatment Plans (ITPs). Provides or arranges for interventions as outlined in each person's ITP, including self-care (personal hygiene & health); social relationships & recreation; education/vocational training; and independent living skills (money management, clothes care & apartment management); teaches skills as outlined in the ITP (including interpersonal & intra-personal skills). Provides med supports. Supports people with skills to support with final goal of living in the community. Documents interventions via the person’s record.
4. Links people to all available resources including entitlements, mental and physical health services, employment services, crisis intervention services, etc. Plans and provides crisis interventions in order to prevent unnecessary psychiatric and somatic and somatic hospitalizations. Encourages each individual to work toward their recovery. Offers support, hope, encouragement, compassion and empathy to individuals who are struggling with their recovery.
5. Provides indirect services to residents, including service coordination; reassurance to the person and to those listed on the person’s Release of Information; shares information with other providers with authorization; participates in indirect services when necessary. Assists people to connect with their community and peers.
6. Provides informal support and socialization to residents whenever possible. Completes all records and documentation requirements including the ITP, Daily Case Notes, Daily Progress Notes, Health Status Reports, and other documents required by the agency.
7. Completes all requirements at the end of the shift including: a) updating bulletin board and documentation hand-off/shift notes; b) checking and responding to voicemail; c) returning and charging cell phone; d) verbally updating next shift; e) when driving an agency vehicle, ensures its cleanliness and refuels as necessary; f) completing timesheets and mileage sheets, when due.
8. Attends meetings as required. Trains new staff and interns the job duties of the team’s HSW staff. Focuses on meeting all COMAR, CARF, HIPAA and other regulatory requirements. Attends certification/recertification training as scheduled. Completes all other training as scheduled/ assigned. Understands role as an essential employee.
9. Performs other tasks related to the overall scope of the job as assigned and shares time between crisis/respite facility and residential facilities.

Qualifications for this Position:
1. High School Diploma or GED equivalent.
- Associate’s, Bachelor’s, or Master’s degree allows for annual education stipend.
2. Ability to provide direct supervision to Human Service Workers as assigned.
3. Strong interpersonal skills.
4. Good documentation skills.
