PEER CARE COORDINATOR

POSITION OBJECTIVES

The Peer Care Coordinator assists individuals in detox using language of the heart in achieving their person and family-centered recovery and wellness goals while using effective and collaborative communication within a multidisciplinary team. The Peer Care Coordinator also works cooperatively with external community service providers and the patient's natural support. The Peer Care Coordinator consistently monitors detox patient service referrals, tracks discharge appointments, and manages health information exchange.

ESSENTIAL FUNCTIONS

  • Ensure quality internal and external customer care and service.
  • Demonstrate flexibility as a team member and promote a positive working environment.
  • Maintain knowledge of all services and programs offered by Gateway.
  • Provide services to individuals in accordance with the guiding principles and core competencies for care coordination best practices, as outlined by LSF.
  • Understand the concepts of recovery and wellness for patients experiencing substance use, mental health, and co-occurring disorders.
  • Provide all services according to insurance, DCF licensing, managing entity, SAMHSA, and CARF accreditation guidelines and document all services within the required timeframes according to organizational policies and procedures.
  • Be able and willing to drive a company vehicle to facilitate patient transportation.
  • Develop partnerships within the community to address service or resource gaps.
  • Integrate referral source, community, and family support into treatment when clinically appropriate, assuring Release of Information is complete in accordance with HIPAA, 42 CFR Part 2, and other federal and state laws.
  • Maintain high ethical conduct regarding dual relationships, professional standards, and organizational values.
  • Provide services with cultural and linguistic competency.
  • Maintain an active caseload providing interventions as needed and within areas of expertise and limits of credentials.
  • Complete a needs assessment and care plan to address each patient's care coordination needs i.e., behavioral health, physical health, social, housing, education, employment, financial stability, and interpersonal problems.
  • Complete Evidenced Based Assessments, which link high-risk patients with a clinical or medical professional based on assessment outcomes.
  • Complete all required documents, including ROIs and Authorization for Info to be Released form, and submit to medical records to gather previous medical and treatment records.
  • Must ensure that patients with an Emergency Department visit, hospitalization, or Crisis Stabilization Unit admission, receive follow-up post-discharge from these services.
  • Provide psychoeducation and support as needed by accessing and collecting relevant information.
  • Engage and retain patients in the treatment program through relationship building and continual positive communication.
  • Involve the patient's natural supports, including family, friends, sponsor, and other community members, as requested and as appropriate.
  • Determine patient progress and needs by reviewing clinical and medical evaluations, treatment objectives, and plans.
  • Collaborate and coordinate with the patient's treatment team, including external providers and distribute information as appropriate.
  • Participate in program staff meetings, medical staffing, and other inter-organizational collaborations.
  • Support patients transitioning from treatment services to the community by reviewing treatment progress and coordinating with the multidisciplinary team and other case participants.
  • Other duties as assigned.

EMPLOYMENT STANDARDS

  • Must be eligible through DCF and AHCA with Level II Background checks throughout employment.
  • Must abide by the organization's Code of Conduct, policy and procedures, and state certification code of ethics.

  • Must complete all in-person and Relias trainings assigned during probation and throughout employment.
  • Must practice universal precautions as part of regular job duties whenever applicable.
  • Timely and regularly complete all required documentation, reports, and other organization-related paperwork.
  • Must be able to perform assigned work independently with minimal supervision and make decisions using appropriate judgment.
  • Maintain the work area clean and orderly, ensuring safe operating conditions.
  • Must be able to take direction and guidance from managers and supervisors respectfully and function as an effective team member.
  • Must have the ability to comply with a workplace of dignity and respect.

QUALIFICATIONS

EDUCATION/EXPERIENCE/

CERTIFICATION/LICENSURE

  • Minimum 2 years of personal recovery experience regarding mental illness/substance abuse and willing to talk to others about their experience.
  • Bachelor's Degree in Counseling, Social Work, or other related human services field.
  • Experience in substance use and/or mental health disorder programs is preferred.
  • Valid driver's license with a safe driving record to meet standards for the organization's insurance requirements.
  • Complete AHCA-approved 24 hours of Targeted Case Management CEUs within 90 days of the hire date.
  • Certification: Certified Recovery Support Specialist and/or Certified Recovery Peer Specialist by the Florida Certification Board
  • Trainings: SOAR, WRAP preferred

SKILLS

  • Strong time management and organization skills.
  • Excellent communication skills.
  • Ability to recognize and respond to emergencies and life-threatening situations appropriately.
  • Ability to work in a fast-paced environment
  • Ability to relate to adolescents, adults, families, referral sources, and internal Gateway team members.
  • Must possess requisite competencies to effectively understand and provide appropriate services to the patient population served.

KNOWLEDGE

  • Knowledge of the recovery process and 12-step programs preferred.
  • Knowledge of adolescent and adult behavior regarding family systems preferred
  • Knowledge and understanding of cultural competencies to establish and maintain strong working relationships with culturally diverse patients, their families, caregivers, and a wide range of community agencies and organizations.
  • Knowledge of referral sources preferred
  • Moderate computer skills, such as inputting 35 wpm, using Microsoft Office Suites, and an Electronic Health Record.
