Gateways Hospital and Mental Health Center, located in Los Angeles, is a non-profit organization that provides an array of comprehensive mental health services for Children, Adolescents, Transitional Aged Youth, and Adults suffering from mental illness in the Greater Los Angeles area. The organization consists of a standalone fifty-five (55) )-bed hospital and six outlying outpatient facilities that have anywhere from fourteen (14) beds to one hundred and thirty-four (134) beds. The organization has approximately four hundred (400) employees.

Summary Of Position

The Case Coordinator is responsible for providing case management services to adults with serious mental illness. Budget adherence, Submission of reports, chart files, group activities and facilitation of oversight entities (including ACOF, HACoLA, LAHSA, DMH, Shelter Care Partnership.)

Essentials Duties

  • Maintains accurate and up-to-date charts with appropriate and timely completion of all required documentation, including but not limited to, resident Intake Screening, Individual Program Plans, case notes, release plans, and housing packets.
  • Meet with clients on a monthly basis to address progress made toward Individual Service Plan goals. At least one chart note a month will be written to document client’s progress.
  • Provides clients with appropriate referrals as necessary, and follow up with clients on progress made with referrals.
  • Assists clients with processing yearly housing recertification so that all clients are recertified by the deadline set by HACoLA.
  • Collects accurate needed paperwork including signed service forms for HACoLA and LAHSA from residents.
  • Completes with accuracy all once a month subsidy reports to ACOF (due on the 3rd of the month.)
  • Maintains communication with team members throughout the shift to assure that all tasks are covered, and offers assistance where needed.
  • Attends some evening community meetings when requested.
  • Completion of two APR reports (one due to HACoLA and one due to LAHSA) which are due yearly and submitted in a timely manner.
  • Responds to in-house crisis as needed and assists other residential staff when the situations arise.
  • Plans and executed monthly outings with residents via transportation in the company van.
  • Is trained on the HMIS. Inputs all milestones for each resident. Charts at least one chart note a month on each client. Does quarterly reports for each client in the HMIS.
  • Observes and records client’s behavior and activities, monitors hygiene. Reports any unusual or concerning behavior to the appropriate staff members.
  • Coordinate and lead two- three mental health rehabilitation groups on weekly basis.
  • Monitors that clients are compliant with outpatient mental health care. Follows up as needed.
  • Initiates Incident Reports when an emergency occurs. Participates in the delivery and investigation of incident reports and all emergency training.
  • Responds appropriately to emergency or problematic situations; i.e. notifies immediate supervisor, staff, administration, police, fire department, ambulance, and paramedics as appropriate.
  • Assures that LAHSA Service Plans are completed twice a yearly before the end of the client’s six month anniversary date.
  • Provides good role model for residents in terms of reliability, social skills, professionalism, tact, leadership, grooming, and integrity.
  • Responds appropriately under difficult or stressful situations.
  • Obtains provisions for various activities including all shopping, picking up of provisions, and obtaining the monies for them from the management company each month.
  • Able to work effectively with a set budget and operate effectively within established guidelines.

Essential Skills

  • Ability to work and relate effectively with multidisciplinary treatment team, clients and public
  • Ability to assess patients for dangerousness and intervene in a timely, calm, appropriate manner
  • Ability to communicate effectively in English, both verbally and in writing.
  • Ability to work independently, use good judgement, and remain calm in crisis situations.
  • Knowledge of basic counseling skills. Understanding of individual, group, and family dynamics.
  • Ability to work indoors and outdoors (i.e. take clients on outings and transportation of supplies)
  • Ability to remain calm, objective, and dependable in situations where residents are emotional or uncooperative.
  • Computer skills in word processing for creating and typing reports or other related documents.
  • Crisis intervention for assessing and de-escalating assaultive behavior.
