Key Responsibilities:
• Conduct thorough assessments of individual nutrition needs by reviewing dietary histories, medical records, and collaborating with the healthcare team.
• Develop tailored nutrition care plans addressing dietary requirements, meal patterns, and enteral or parenteral feeding compositions.
• Collaborate with healthcare professionals to establish patient care goals.
• Document initial assessments and progress in medical records according to established standards.
• Evaluate patients' nutritional statuses and suggest optimal care strategies.
• Provide counseling to individuals, families, and caregivers on normal nutrition and therapeutic diets, considering varied backgrounds.
• Conduct group nutrition classes as necessary.
• Write nutrition-related orders for hospitalized patients following Clinical Nutrition protocols.
• Advocate for individuals and groups on nutrition-related issues and available resources.
• Assist in planning for continued nutrition care post-discharge using ancillary resources.
• Facilitate referrals and implement quality assurance activities to ensure outstanding service outcomes.
• Utilize electronic tools for practice documentation, research, and education.
