Key Responsibilities
- Deliver essential patient care and services as directed by the registered nurse within the assigned unit.
- Adhere to the registered nurse's guidance to fulfill patient care tasks in accordance with established clinical protocols.
- Accurately gather and document patient data to support assessments and ensure the integrity of medical records, reporting any abnormalities to the RN.
- Identify deviations from expected patient outcomes and communicate findings to the RN.
- Recognize emergency situations and respond appropriately.
- Reinforce patient education under the delegation of the RN.
- Establish and maintain a therapeutic rapport with patients through effective communication.
- Proficient in basic patient care skills.
- Strong verbal and written communication abilities.
- Detail-oriented with a focus on accurate data documentation.
- Ability to follow clinical procedures diligently.
- Skilled in recognizing and responding to emergency situations.
- Empathetic with excellent therapeutic communication skills.
- High School Diploma or equivalent.
- Valid Wisconsin CNA Certification permitting work in Federally Certified Home Health Agencies and Hospices.
- Previous experience engaging with the public is mandatory.
- Current and valid Wisconsin Driver’s License along with a reliable and insured vehicle.
