Job Overview

Join Horizon’s esteemed Inpatient Hospice facilities - Lawlis Family Hospice and Kathy’s Hospice - where we uphold a tradition of celebrating the human spirit through exceptional patient care. We are currently seeking a dedicated and compassionate Certified Nursing Assistant (CNA) for a full-time, third shift opportunity (11:00 PM - 7:30 AM) across both facilities.

Key Responsibilities

• Deliver essential patient care and services as directed by the registered nurse within the assigned unit.
• Adhere to the registered nurse's guidance to fulfill patient care tasks in accordance with established clinical protocols.
• Accurately gather and document patient data to support assessments and ensure the integrity of medical records, reporting any abnormalities to the RN.
• Identify deviations from expected patient outcomes and communicate findings to the RN.
• Recognize emergency situations and respond appropriately.
• Reinforce patient education under the delegation of the RN.
• Establish and maintain a therapeutic rapport with patients through effective communication.

• Detail-oriented with a focus on accurate data documentation.
• Ability to follow clinical procedures diligently.
• Skilled in recognizing and responding to emergency situations.

Qualifications

• High School Diploma or equivalent.
• Valid Wisconsin CNA Certification permitting work in Federally Certified Home Health Agencies and Hospices.
• Previous experience engaging with the public is mandatory.
• Current and valid Wisconsin Driver’s License along with a reliable and insured vehicle.
