Generation Hope is a Recovery Community Organization (RCO) dedicated to empowering individuals in recovery through peer support, workforce development, and advocacy. Our Peer Recovery Program provides culturally responsive services to support individuals on their recovery journey. We are seeking an Intake Coordinator to facilitate client admissions and ensure seamless program operations.
Position Summary
The Intake Coordinator is responsible for managing the intake process for clients entering the Peer Recovery Program. This role involves conducting initial screenings, verifying documentation, scheduling appointments, and ensuring clients are connected to the appropriate services. The Intake Coordinator will work closely with Peer Recovery Specialists and administrative staff to support program efficiency and client success.
Key Responsibilities:
- Conduct initial screenings and assessments for clients entering the Peer Recovery Program.
- Collect and verify required intake documentation, ensuring compliance with program policies.
- Schedule and coordinate client appointments, including follow-ups and referrals to external resources.
- Maintain accurate client records in the Electronic Health Records (EHR) system.
- Assist with insurance verification and billing-related processes in collaboration with the billing department.
- Ensure clients complete all required intake forms and consents before services begin.
- Communicate effectively with clients, families, and community partners regarding program services and expectations.
- Track and log hours for Peer Recovery Specialists weekly to ensure accurate documentation of service delivery.
- Support program data collection and reporting efforts to meet funding and compliance requirements.
- Collaborate with program staff to improve the intake process and enhance client experience.
- Maintain confidentiality and adhere to HIPAA and other compliance guidelines.
Qualifications:
- Associate’s or Bachelor’s degree in social work, human services, or a related field preferred. Relevant work experience is required.
- Previous experience in intake coordination, case management, or administrative support in a healthcare or social services setting.
- Familiarity with addiction recovery, peer support, and culturally responsive care.
- Strong organizational skills and attention to detail.
- Proficiency in using Procentive and other client management systems.
- Ability to communicate effectively with diverse populations, including individuals in recovery.
- Knowledge of insurance verification and billing procedures is a plus.
- Ability to work independently and as part of a team.
- Bilingual proficiency (Somali or other languages) is a plus.
