We are currently seeking a skilled LPN/LVN to join our team as a long term care professional. The successful candidate will work 40 hours per week, with 8 hour shifts, evenings. This is a travel position with a start date of ASAP and a duration of 8 weeks.
Key Responsibilities:
Provide high-quality patient care in a long term care setting. Collaborate with healthcare team members to develop and implement effective patient care plans. Communicate effectively with patients, families, and healthcare staff. Maintain accurate and up-to-date patient records. Adhere to established policies and procedures.
Required Skills and Qualifications:
Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) degree. Current state licensure required. At least 1 year of long term care experience. Excellent communication and interpersonal skills. Ability to work independently and as part of a team. Physical ability to lift and transfer patients.
