We are seeking a skilled Nurse Practitioner to join our hospice team in Conyers, Georgia. As a key member of our interdisciplinary team, you will be responsible for providing high-quality patient care and supporting the physical, emotional, and spiritual needs of our patients and their families.
Job Summary
- Conduct face-to-face encounters with patients and their families to assess clinical needs and provide supportive care.
- Work closely with our interdisciplinary team to develop and implement comprehensive care plans that meet the unique needs of each patient.
- Participate in ongoing professional training and development to stay current with best practices in hospice care.
Responsibilities:
- Perform physical assessments for face-to-face encounter visits within regulatory time frames established by CMS.
- Provide clinical findings, including review of medical treatment plans, diagnostic tests, lab results, and medication lists to the physician who is recertifying the patient for hospice eligibility.
- Makes appropriate recommendations to the IDG based on findings of the F2F assessment.
