GENERAL JOB DESCRIPTION     

 

Gesher’s Nurse Case Manager, RN will provide discharge planning, coordination of care, and education for participants and stakeholders within the organization.  Promote healthy lifestyles and prevent reoccurring hospitalizations by monitoring, assessing, counseling, supporting, and empowering participants to improve their quality of life and maintain independence in the community and their current living arrangements. 

 

QUALIFICATIONS     

 

Education: 

Licensed by the State of Michigan as a Registered Nurse, minimum requirement. Associates degree required; bachelor’s degree preferred. 

Other:

  1. Two years related work experience with individuals with intellectual, developmental, emotional and/or physical impairments and/or older adults, especially those with cognitive changes and dementia, preferred.
  2. Interpersonal skills sufficient to communicate with participants, guardians, staff, and public.
  3. Demonstrates use of critical thinking, attention to detail, sound clinical judgment and assessment skills for decision making
  4. Experience in Microsoft Office and composition skills sufficient to document patient contact, services provided, and to prepare required reports.
  5. Computer skills sufficient to navigate Gesher’s case management system efficiently with training.
  6. Must have valid driver’s license and own vehicle.
  7. Must possess and maintain valid CPR certification while employed.

 

DUTIES and RESPONSIBILITIES

  1. Train and supervise designated team members in medication administration/assistance and provision of other health measures to participants. 
  1. Review participants’ daily health-related symptoms, administer first aid and/or refer participants to appropriate community resources and follow-up on any medical services required.
  1. Assist in developing participant discharge planning and in the care plan process, communicating participant’s health needs to involved staff.  This includes ensuring follow up with the participant’s primary care physician following hospitalization.  Document health-related interventions performed per agency protocols and in a timely manner. 
  1. Gather and review yearly physicals and maintain database of current medication logs. 
  1. Develop and implement training programs for staff, interns, participants, residents, and volunteers, and as appropriate for caregivers to promote health care maintenance, including personal care and universal precautions as needed.  Such training topics may include medication administration, bedsore prevention, gait belt training, transferring, bathing, seizure response, first aid, CPR, health and hygiene, and other ADL related training. 
  1. Maintain open lines of communication between health providers, nurses, home staff, program staff, and family caregivers to ensure continuity of health care. 
  1. Participate in referral screening process and complete a nursing assess, including review of prior medical records and social work assessment for each new participant in the residential and/or dementia day program. 
  1. Serve on agency committee(s) as appointed and participate in multi-disciplinary case consultations. 

Working Conditions: 

  1. Ability to work a contingent (varied hours) schedule.
  2. Ability to transfer or assist in the transfer of individuals weighing up to 300 pounds.
  3. Ability to perform all physical skills required to deliver care.  Sufficient strength to lift, move and transfer most participants (ambulation, transport, and repositioning) move and carry equipment and perform CPR, which requires sufficient body weight and adequate lung expansion.
  4. Dexterity and mobility sufficient to bend, stoop, and get down on the floor.
  5. Ability to meet residents and participants at the location where they receive services from Gesher.
  6. Flexibility to meet with hospital/healthcare staff virtually, in-person, or via phone upon discharge from medical or mental health treatment facility.  

 

Exempt

This position is exempt from the overtime pay provisions of the Federal Fair Labor Standards Act.
