Medical Case Manager Position
The Medical Case Manager position requires an individual who is organized, empathetic, and comfortable being a member of a team.
The Medical Case Manager enhances the quality of the clients experience by leveraging their knowledge of the medical system and partnering with healthcare providers to ensure clients receive seamless, all-encompassing care, including :
- Educating and empowering clients and their family members to equip them with the resources and assistance they need to conquer their health challenges.
- Identifying and leading clients to community resources and the support they need to thrive.
- Coordinating client care with the assistance and direction of the physician.
- Assess treatment plans and keep impeccable client records (treatment plans, case notes, discharge plans, etc.).
- Partnering with healthcare providers and fellow teammates to make sure every client gets the care they deserve.Attending important client appointments and advocate for them.
- Assisting with hospital discharges as needed, assuring clients return to a safe and functional home environment.
- Establishing treatment programs by setting schedules and routines; coordinating services being provided; and arrange logistical details such as transportation.
- Monitoring clients rehabilitative progress by verifying attendance; observing and evaluating treatments and responses; advocating for needed services and entitlements.
