Key Responsibilities
- Conduct comprehensive non-clinical assessments of members’ functional status, health, behavioral, psychosocial, and community resource needs in accordance with federal regulations.
- Provide critical assessment data to the Interdisciplinary Care Team (ICT) and facilitate care coordination to meet members’ needs.
- Design and implement Individual Care Plans (ICP) and Person-Centered Service Plans (PCSP) tailored to each member's requirements, addressing barriers to care effectively.
- Continually monitor and adjust care plans as necessary to guarantee that members receive essential medical and supportive services.
- Oversee care transitions with clear communication strategies for effective member care and discharge planning.
- Clarify medical benefits, policies, and procedures for members, providers, and community organizations.
- Engage in clinical case reviews to exchange best practices and uphold quality care standards.
- Document assessments and modifications to care plans following established policies and procedures.
- Remain updated on community resources, payer requirements, and available network services for the designated population.
- Collaborate with fellow care managers to identify avenues for enhancing clinical practices and policy development.
- Provide support to other Assessment nurses as required.
- Analyze data to improve care delivery and optimize patient outcomes.
- UAS Certified RN licensed in New York State (New graduates are welcome to apply).
- Bachelor's Degree in Nursing or a related field.
- Valid New York State RN License.
- Knowledge of non-clinical assessments and care plan development.
- Strong communication and collaboration skills to work effectively with care teams.
- Proficiency in data analysis to enhance patient outcomes.
- Bilingual in English/Spanish is preferred, but not mandatory.
- Bachelor's Degree in Nursing or related field is required.
- Valid RN License in New York State.

- Pay Rate: \(58.00 - \)62.00 per hour (commensurate with experience), plus mileage reimbursement.
- Contingent to Permanent: Eligible for permanent placement after 90 days.
- Orientation includes a 5-day training program for UAS certified RNs.
