Key Responsibilities
- Perform non-clinical assessments of members' functional status, health, behavioral, psychosocial, and community resource needs in accordance with federal guidelines.
- Present assessment information to the Interdisciplinary Care Team (ICT) and coordinate care to address members' needs effectively.
- Create Individual Care Plans (ICP) and Person-Centered Service Plans (PCSP) tailored to member needs, addressing any barriers to care.
- Monitor and adjust care plans as necessary to guarantee members receive essential medical and supportive services.
- Manage care transitions with clear communication for effective member care and discharge planning.
- Clarify medical benefits, policies, and procedures for members, providers, and community agencies.
- Engage in clinical case reviews to share best practices and uphold the quality of care.
- Document assessments and care plan updates following established policies and procedures.
- Stay updated on community resources, payer requirements, and network services pertinent to the target population.
- Collaborate with fellow care managers to identify areas for enhancement in clinical practices and policy development.
- Provide backup support to other Assessment nurses as necessary.
- Analyze data to improve care delivery and optimize patient outcomes.
- UAS Certified RN, licensed in New York State (new graduates encouraged to apply).
- Bachelor's Degree in Nursing or a related field.
- Valid NY State RN License.
- Bilingual proficiency in English/Spanish is preferred but not mandatory.
