Key Responsibilities
- Conduct comprehensive assessments to ascertain individuals’ clinical, psychosocial, and personal needs.
- Design and uphold person-centered Life Plans to ensure services are aligned with personal goals and preferences.
- Oversee and facilitate the execution of care plans, making necessary adjustments as circumstances evolve.
- Champion health and wellness by aiding access to care, disease management, and community resources.
- Advocate for individuals' needs while fostering their independence and self-advocacy.
- Collaborate with families and natural support networks to promote a cohesive care process.
- Maintain precise records utilizing Health Information Technology, ensuring compliance with confidentiality and HIPAA standards.
- Deliver culturally and linguistically appropriate services to individuals and their families.
- Proficient time management, case management, and communication skills.
- Capability to effectively manage crisis situations.
- Basic technology proficiency and familiarity with health records.
- Fluency in Spanish is essential.
- Bachelor's degree with a minimum of two years of relevant experience, OR
- RN license with two years of experience in any capacity (not limited to case management), OR
- Master's degree with at least one year of pertinent experience.

- Competitive pay range: \(22.50-\)24.50 per hour.
