Key Responsibilities
- Conduct comprehensive assessments to evaluate individuals' medical, social, and emotional requirements.
- Develop and uphold person-centered Life Plans tailored to each individual's distinct health and wellness objectives.
- Identify and address barriers to care, including social determinants of health.
- Ensure health promotion and care coordination to connect individuals with essential medical and community resources.
- Facilitate referrals to healthcare providers, social services, and community initiatives.
- Assist individuals with chronic disease management and ongoing care needs.
- Advocate for self-direction, empowerment, and independence.
- Ensure that services provided are culturally and linguistically relevant.
- Maintain precise documentation and adhere to all applicable regulations, including HIPAA.
To thrive in this position, candidates should possess:
- A Bachelor’s degree accompanied by a minimum of two years of relevant experience, or a Registered Nurse (RN) license with two years of professional experience, or a Master's degree with at least one year of relevant experience.
- Fluency in Spanish (Required).
- Previous experience in case management, advocacy, or direct care is an asset.
- Strong time management, organizational, and interpersonal communication skills.
- The ability to assess crisis situations and respond promptly and effectively.
- Basic proficiency in technology for documentation and health records.
- Bachelor's degree in a relevant field.
- RN license or Master's degree may also meet the qualifications.
