Key Responsibilities
- Conduct thorough assessments to identify the medical, social, and emotional needs of individuals.
- Design and uphold a personalized Life Plan that ensures individuals receive crucial healthcare and community resources.
- Coordinate healthcare and social services to guarantee the provision of appropriate care.
- Advocate for individuals and families, empowering them to navigate available resources effectively.
- Collaborate with healthcare professionals and community organizations to provide comprehensive support.
- Utilize Health Information Technology for accurate documentation and effective communication among care teams.
- Ensure that all services are delivered in a culturally and linguistically sensitive manner.
- Adhere to state and federal regulations, Medicaid guidelines, and organizational policies.
- Fluency in Spanish (Required).
- Strong skills in case management and time management.
- A genuine passion for assisting individuals with intellectual and/or developmental disabilities in reaching their goals.
- Ability to work autonomously while managing multiple tasks efficiently.
- Basic proficiency in technology for record-keeping and communication.
- A Bachelor's degree along with two years of relevant experience, OR
- A Registered Nurse (RN) license with two years of experience (experience does not necessarily need to be in case management), OR
- A Master's degree with one year of relevant experience.
