Key Responsibilities
As a Care Manager, you will engage with individuals to coordinate essential care and services. Your primary responsibilities will include:
- Conducting thorough assessments to determine clinical, social, and personal needs
- Developing and continually updating a person-centered Life Plan that encompasses healthcare, social services, and long-term support
- Monitoring the effectiveness of the Life Plan to facilitate ongoing improvement in care
- Addressing social determinants of health and advocating for the needs of individuals
- Facilitating access to medical, social, and community resources
- Collaborating with interdisciplinary teams to ensure comprehensive support
- Ensuring services are culturally and linguistically attuned to the needs of individuals
- Promoting self-advocacy and independence among clients
- Maintaining compliance with HIPAA and other regulatory standards
- Accurately documenting all services and interactions in a timely manner
The ideal candidate must possess strong problem-solving capabilities, excellent communication skills, and effective time-management abilities. Qualifications
- Bachelor’s degree with two years of relevant experience, OR
- A Registered Nurse (RN) license with two years of relevant experience, OR
- A Master's degree with one year of relevant experience
- Experience working with individuals with intellectual and developmental disabilities is required
- A valid driver's license is necessary

- Competitive Pay: \(22.50 - \)24.50 per hour
- Hybrid remote position based in Syracuse, NY
- Monday-Friday schedule (8-hour shifts, no weekends, no on-call!)
