Key Responsibilities
- Conduct thorough assessments to identify individual needs and preferences.
- Develop and uphold a personalized Life Plan that aligns with each individual's health and life aspirations.
- Facilitate care coordination and advocacy to ensure clients receive the necessary support.
- Connect individuals with healthcare providers, community resources, and social services.
- Collaborate with a multidisciplinary team to deliver comprehensive, person-centered care.
- Assist in health promotion and chronic illness management.
- Provide culturally and linguistically appropriate resources to individuals and families.
- Maintain confidential documentation while ensuring compliance with HIPAA regulations.
- Bilingual in Spanish (Required).
- Strong abilities in case management, time management, and advocacy.
- Proficient in assessing needs, crafting care plans, and linking individuals to resources.
- Basic technological skills for documentation and communication.
- Bachelor's degree with a minimum of 2 years of relevant experience, OR
- Registered Nurse (RN) license with at least 2 years of experience, OR
- Master's degree with a minimum of 1 year of relevant experience.
