This pivotal position allows you to provide invaluable indirect patient care and support for both patients and their caregivers. You will be responsible for managing after-hours calls and supervising weekend/on-call clinical teams, with a primary focus on delivering exceptional patient care and customer service outside of regular business hours. While primarily remote, occasional in-person educational or in-service engagements may occur. Your responsibilities will include after-hours administrative duties during weekdays and weekends, in addition to 16 hours of scheduled management tasks over the weekend.

Key Responsibilities
- Deliver patient and caregiver education concerning diagnoses, medications, infection control, and safety protocols in accordance with the care plan.
- Act as the administrative on-call point of contact during after-hours and weekends, including participation in weekend holiday rotations.
- Supervise weekend management, ensuring productivity and admissions flow smoothly.
- Promote collaboration across healthcare disciplines to enhance patient outcomes.
- Ensure compliance with state, local, and payer regulations, aligning documentation with the provided care to meet regulatory standards.
- Oversee requisition and supply management, promoting patient independence through appropriate equipment and supplies.
- Exhibit proficiency in therapeutic interventions and maintain competency across all levels of patient care.
- Provide leadership and support to weekend clinical teams, ensuring adherence to best practices.
- Educate patients and caregivers on essential safety measures related to care.
- Recognize and document any changes in patient status promptly, ensuring compliance with agency and regulatory requirements.
- Demonstrate proficiency in OASIS documentation, maintaining required timelines and accuracy.
- Report incidents in accordance with agency and regulatory incident management standards.
- Uphold patient confidentiality, adhering strictly to patient rights and agency policies.
- Participate in staff meetings, patient care conferences, in-services, and other relevant meetings as needed.
- Undertake additional responsibilities as assigned, all within the scope of practice.
- Degree in Nursing from an accredited institution.
- A valid nursing license in the state of practice.
- Current CPR certification.
- Prior experience in home care is preferred.
- Strong clinical acumen and a professional demeanor.
- Proficient in computer usage, with an aptitude for learning industry-specific software.
- Exceptional written, oral, and interpersonal communication skills.
- Robust organizational and time management skills, with a keen attention to detail.
- Ability to function effectively within a team environment and collaborate with other professionals.
- Capacity for critical thinking, effective research, and problem-solving.
- Willingness to pursue educational objectives during the orientation process.
