Key Responsibilities
- Manage a caseload of post-release services, providing ongoing support through assessments, planning, and psychoeducation.
- Conduct home studies and comprehensive evaluations to ensure the safety, well-being, and appropriate placement of children.
- Collaborate closely with care providers, community resources, and sponsors to identify and address needs, risks, and additional support opportunities.
- Maintain consistent communication via telephone, virtual meetings, and in-person visits with children and their sponsors.
- Employ trauma-informed approaches and therapeutic techniques in client interactions.
- Ensure timely and accurate documentation while safeguarding confidential and sensitive information.
- Report any concerns related to children, sponsors, or significant incidents as mandated.
- Empathetic approach towards case management.
- Proficient communication skills, both verbal and written.
- Attention to detail with the ability to navigate sensitive situations effectively.
- Knowledge of family preservation, kinship care, and trauma-informed practices.
- Bilingual proficiency in English and Spanish (both written and verbal).
- Competence in Microsoft Office and data entry systems.
- Valid driver’s license and availability for national travel as necessary.
- A Bachelor’s degree in Social Work, Education, Psychology, Sociology, or a related field.
- Minimum of 1 year of case management experience, including the ability to write professional assessment reports.
- Familiarity with principles of family preservation, kinship care, and trauma-informed care.
