We invite you to join our dedicated team as a Case Manager Level II, where your passion for supporting children and families will be vital in our Post Release Services program. This remote position offers a unique opportunity to create a positive impact by ensuring the safety and well-being of children and their sponsors during critical transitions.

Key Responsibilities

  • Oversee a caseload of post-release services cases, emphasizing family preservation and child welfare.
  • Conduct thorough home assessments to evaluate safety and suitability for placements.
  • Deliver continuous case management support, including assessments, planning, and educational resources for both children and their sponsors.
  • Partner with local resource providers to gather information necessary for comprehensive evaluations.
  • Implement trauma-informed care and short therapeutic interventions to meet the needs of families.
  • Maintain regular communication through virtual and in-person home visits.
  • Ensure meticulous and confidential documentation compliant with program standards.
  • Act as a mandated reporter, addressing any signs of abuse or neglect.
  • Engage in ongoing training and quality improvement efforts.
  • Travel nationally for in-person visits, which may include evenings and weekends.

Qualifications

  • Bachelor's degree in Social Work, Education, Psychology, Sociology, or a related discipline.
  • Minimum of 1 year of case management experience.
  • Proficiency in crafting detailed assessment reports.
  • Experience with immigrant communities and trauma-informed practices.
  • Familiarity with family preservation, kinship care, and child welfare regulations.
  • Bilingual proficiency in English and Spanish (both verbal and written).
  • Competent in Microsoft Office and data entry systems.
  • Valid driver's license and willingness to travel nationwide.
