Key Responsibilities
- Manage a designated caseload of children and their sponsors, delivering essential post-release services and assessments.
- Conduct thorough home study reports to evaluate the safety and suitability of potential placements for children.
- Collaborate effectively with care providers to ensure that sponsors receive the necessary resources and support.
- Assess and verify the sponsor’s ability to provide a safe and appropriate environment for the child.
- Maintain consistent communication with children and sponsors through virtual and in-person home visits.
- Employ trauma-focused methodologies when working with children and their sponsors, offering brief therapeutic techniques when required.
- Deliver ongoing case management services including assessments, planning, and psychoeducation.
- Document all case activities meticulously while upholding confidentiality standards as per program regulations.
- Report any significant concerns or events to the appropriate authorities and act as a mandated reporter for cases of abuse or neglect.
- Participate in training sessions and meetings, contributing to the ongoing enhancement of case management practices.
- Travel as necessary to conduct in-person home visits, which may include evenings and weekends.
- Proven ability to actively listen and demonstrate empathy in various situations.
- Aptitude for asking insightful questions to discern the unique needs of clients.
- Capability to build trust and rapport with individuals from diverse backgrounds.

- Familiarity with family preservation, kinship care, child welfare practices, and relevant policies.
- Strong communication abilities for engaging with families, caseworkers, and community partners.
- Discretion in handling sensitive and confidential information.
- Fluency in both English and Spanish, with strong oral and written skills.
- Proficiency in Microsoft Office and data entry systems.
- A valid driver's license and readiness to travel for in-person visits as necessary.
