You will engage with a diverse group of patients, particularly those managing chronic conditions, ensuring they receive the personalized, continuous care they deserve.
Key Responsibilities
- Conduct monthly coordination calls with patients, healthcare providers, and pharmacies to facilitate holistic care between office visits.
- Collaborate with medical practices to create and regularly update individualized care plans.
- Identify and tackle social determinants of health while addressing gaps in care to ensure patients receive essential referrals and services.
- Empower patients to take an active role in managing their health and well-being by offering clear, actionable guidance within their care plans.
- Make 30-50 calls daily, engaging with at least 15 patients to deliver approximately 20 minutes of care coordination services.
- Uphold patient privacy and confidentiality in all interactions, strictly adhering to healthcare regulations.
- Minimum of 1 year of experience as a Certified or Registered Medical Assistant (CMA/RMA).
- Proficiency in both Spanish and English, with outstanding verbal and written communication skills.
- Comfortable navigating multiple EMR/EHR systems, Microsoft Office, and Google Suite.
- Strong ability to think on your feet and engage patients with professionalism and empathy.
- Excellent critical thinking skills to effectively address patient inquiries and needs.
- Self-driven and capable of working independently in a vibrant, fast-paced virtual environment.
- Experience in call centers, triage, or care coordination is a beneficial asset.
- Familiarity with geriatrics or case management will be considered advantageous.
- Certification or registration as a Medical Assistant (CMA/RMA) is mandatory.
